This page includes the following forms:
- Pack 365 Flag Ceremony - This is the ceremony used at pack meetings
- Activity and Consent Form - Recommended to be used for all activities that are outside of regular den and pack meetings
- Annual Health and Medical Record - Required for all participants (including parents, guardians, and siblings) for any scouting activities such as local tours and camping trips less than 72 hours. Only needs to be filled out once a year and will be kept on file.
