For New Scouts



The Scouting year runs from June 1 – May 31. New cubs may join at any time, but most join at the beginning of the school year. For a new Scout to join Pack 149 please do the following:

1.  Please complete the following forms and provide copies to the Pack 149 Membership Chair listed below:

a)  BSA - Online Youth Application.  If you use this new online process, please keep a copy of the application for your records. You do not need to submit a copy to the Membership Chair. If you choose to complete and submit an application other than through the online process, please use the PDF document provided here: Cub Scout Youth Application and email a copy to Membership Chair Adrianne Bosco at the email address provided below.    

b)  Annual Health and Medical Record. Complete Parts A and B.  

Please deliver completed health forms to Pack 149 Membership Chair Adrianne Bosco by email (adriannelynn@yahoo.com ) or at 2217 N. Quincy St, Arlington VA, 22207. 

2.  Complete the following forms online:

a)  Emergency Contact form
b)  Vehicle and Insurance form  
c) 
Picture Authorization form 

3. Submit $80 payment via PayPal (this includes a Boys Life subscription):

Notes:

All new Scouts will receive a free Scout uniform neckerchief and Pack "149" uniform shoulder numerals.

For Returning Scouts


The Scouting year runs from June 1 – May 31.  Each year, returning Pack 149 scouts must do the following:

1.  Download and complete the following paper-based form.
a)  Annual Health and Medical Record.  Fill out Parts A and B.  
Once completed, make and keep copies for yourself.  Deliver to Rita Olsen by email (rita.dicasa@gmail.com) or at 3827 Lorcom Lane, Arlington VA, 22207.

2.  If any of the info below has changed, complete the following forms online.
a)  Emergency Contact form
b)  Vehicle and Insurance form  
c) 
Picture Authorization form

3.  Submit $80 payment via PayPal.

Notes:
Unlike in years past, you MUST complete a new Annual Health and Medical Record each year.  This is important from a safety and liability perspective.  Please see this link if you have questions about why this is important and how this information is used.
The only exception is - Returning scouts who completed an Annual Health and Medical Record for Goshen or Camp Snyder over the summer need not complete this form in the Fall as the form is good for 12 months.
Scouts who do not submit a new Annual Health and Medical Record by September 30 will not be able to participate in Scouting events.

For Adult Leaders



1. Download and complete the following paper-based form.
a)  BSA Adult Application
Once completed, make and keep copies for yourself. Deliver to Rita Olsen by email (olsenac21@yahoo.com) or at 3827 Lorcom Lane, Arlington VA, 22207.


Notes:
-  The Pack pays the Adult Leader fees.
All new Adult Leaders will receive free "149" uniform shoulder numerals.
