This page will allow you to access Health Forms, Permission Slips, and other files that you may need from time to time.  Please check back regularly for updates.

The file titled "Medical Form parts_ab_udated 2013" is the medical form, which is an editable PDF file.  Please fill out in it's entirety.  As an updated form is needed yearly, it is easiest to do a "save as" for each family member who participates in the activities and fill in.  That way you need only fill it out once and can just make any adjustments yearly, print, and sign.

The "Pack27PermissionSlip" file is exactly that - an editable PDF file permission slip that you can fill out as needed.  A permission slip is necessary for each child under 18 attending an activity.

"Parent-Pledge-and-Cub-Scout-Promise" (PDF file) outlines everything needed for a new Scout to earn their Bobcat Badge.
